
Extended-Release Formulation
An extended-release formulation is a medication that releases its active ingredient slowly over many hours instead of all at once. This often means one tablet a day is enough, and the drug level in the blood stays more even.
A normal tablet dissolves quickly in the stomach. The active ingredient, meaning the part of the medication that actually has an effect, then enters the bloodstream within a short time. The level in the body rises steeply and then falls again afterward. An extended-release formulation is built precisely so that this does not happen. It releases the active ingredient in portions over hours, similar to a watering can with very fine holes. The English term literally means “prolonged release”; in German this is also called Retardform.
Why one tablet a day often works better than three
Many active ingredients only help within a certain range. If too little is in the blood, nothing happens. If too much is present, side effects occur. A fast-releasing tablet briefly shoots above this range and later drops below it. So one first experiences strong side effects and then a gap in effectiveness.
An extended-release formulation, by contrast, keeps the level flatter and more even. This is especially important for painkillers, blood pressure medications, or ADHD medications. In ADHD, a child is meant to stay focused throughout the entire school day, not just until the third period.
On top of that, there is a very practical point. Anyone who has to take a tablet three times a day reliably forgets the middle one. Experts call this treatment adherence. A single tablet in the morning is much more likely to be taken, and this often determines whether a treatment works at all.
What happens inside the tablet
The simplest design is the matrix tablet. Here, the active ingredient is embedded in a scaffold made of a swelling plastic or wax. In the stomach, this scaffold forms a viscous gel layer. The active ingredient has to slowly work its way outward through the gel, and this is exactly what takes time.
A second design works with a shell. The tablet is coated with a thin membrane that has tiny pores. There are even variants with a laser-drilled hole: water penetrates, pushing the active ingredient out like from a syringe. Such osmotic systems release almost constantly over twelve hours, regardless of what one has eaten.
A common misconception is that such tablets may be split or chewed. This destroys the brake mechanism. The entire daily dose is then released at once, which can be life-threatening with potent active ingredients. That is why such packages explicitly state that they must be swallowed whole. The extended-release form should not be confused with enteric-coated tablets: these also release their active ingredient with a delay, but only until reaching the intestine, after which it is released all at once.
How to recognize ER products in the pharmacy and in financial news
On the shelf, these medications can be recognized by abbreviations in the product name. Common ones are ER, XR, SR, LA, or the German word “retard” after the brand name. Well-known examples include painkillers for cancer patients, metformin products for diabetes, and ADHD medications such as methylphenidate in its XR variant.
In financial news, the term appears for a different reason. When the patent on an active ingredient expires, other companies are allowed to copy it cheaply. A new extended-release version of the same active ingredient, however, can be protected again. This strategy is referred to as lifecycle management and is regularly a subject of patent disputes.
Regulatory agencies such as the American FDA also review such formulations separately. A company must demonstrate that the release truly functions as intended in the body and that the active ingredient cannot be dissolved abruptly through alcohol or crushing. Reports of delayed approvals or recalls of ER products can therefore occasionally move stock prices as well.
