Heart Rate Variability

Heart rate variability describes how much the time intervals between two heartbeats fluctuate. It is considered an indicator of how well the body adapts to exertion, stress, and recovery, and is now measured by many smartwatches and fitness rings.

A heart never beats completely evenly. Even when sitting calmly and the heart beats sixty times per minute, the intervals between individual beats vary in length. Sometimes there’s one second in between, sometimes 0.95 seconds, sometimes 1.05 seconds. This very fluctuation is called heart rate variability, often abbreviated as HRV. It is not a flaw or a sign of illness, but normal. A heart that beats like a metronome in exactly the same rhythm is actually considered a bad sign.

What the fluctuation reveals about the body

The rhythm of the heart is controlled by two opposing forces in the nervous system. One part accelerates and prepares the body for exertion. The other part slows things down and switches to recovery mode. Both are constantly pulling on the heartbeat at the same time, sometimes one more strongly, sometimes the other. This ongoing fine-tuning creates the fluctuations that are measured as HRV.

This gives rise to the common interpretation. High variability suggests that the body can react flexibly and is well recovered. Low variability often occurs with stress, lack of sleep, infections, or after hard training. Athletes use this to decide whether to train intensely today or rather take a break. In medicine, a persistently very low HRV is a warning sign, for example after a heart attack.

It is important to distinguish this from resting heart rate. Resting heart rate tells you how often the heart beats per minute. HRV tells you how regularly it does so. Two people can have the same pulse of sixty and still show completely different variability. The absolute values are also very individual, depending heavily on age and predisposition. It therefore only makes sense to compare with one’s own values from last week, not with those of friends.

How heartbeats become a number

First, you need the exact timing of each individual heartbeat. In a hospital, this is provided by an ECG, a device that records the electrical signals of the heart via electrodes on the skin. Watches and rings work differently. They shine LEDs into the skin and measure how much light comes back. With every heartbeat, the amount of blood in the tissue changes, and with it the reflected light.

This series of measurements produces a list of intervals, for example 1000, 960, 1040 milliseconds. The most common metric is called RMSSD. In simplified terms, it describes how much two consecutive intervals differ from each other on average. Typical values range between 20 and 100 milliseconds. There are also methods that break down the measurement series into slow and fast fluctuations and compare their proportions.

The measurement is sensitive. Movement, a loosely fitted wristband, or cold fingers interfere with the light signal. Breathing, alcohol the night before, and the time of day also significantly alter the result. That’s why most devices measure at night during sleep, when you lie still. Individual outliers say little; only a trend over several days is meaningful.

HRV in watches, apps, and studies

Almost every modern smartwatch and every fitness ring now displays HRV values. They often hide behind names like recovery score, readiness, or Body Battery. Behind this lies software that combines nightly HRV with resting heart rate, sleep duration, and training data into a single score. Manufacturers usually don’t disclose exactly how this calculation works.

In the tech industry, HRV is also interesting because it is one of the few numbers that a wrist-worn device can continuously and unobtrusively capture. Companies use such data to train models intended to detect infections, periods of stress, or sleep problems early on. At the same time, this is very personal health data, which regularly raises questions about data privacy.

A common misconception is that HRV is a direct stress meter. It is only an indirect signal, and many causes lead to the same downward swing. A low value can come from a cold, from too little sleep, or simply from an inaccurate measurement. As a rough trend over weeks, the number is useful; as a daily assessment of one’s own condition, it should be taken with caution.
