Cadence is a clinical AI platform for managing chronic diseases (including hypertension, heart failure, and diabetes) in older patients. Supervised AI agents continuously monitor vital signs, symptoms, medication, and engagement, automatically resolve routine cases, and escalate complex cases to physicians. The platform is directly integrated into the EMR systems of partner health systems and is deployed across three care models (preventive primary care, remote monitoring, post-acute care). Cadence works with over 20 U.S. health systems and manages more than 100,000 patients; in June 2026, the company secured a Series C funding round of $100 million led by Spark Capital.
